The Diabetes Center at Mercy can never replace the services of a primary care physician. We can provide the critical support needed for a patient faced with the life-changing prospect of managing diabetes. We can also provide access to clinical trials, needed equipment and specialized education. We are here to help you help your patient best.

Introduction and Uses of CGMS in the Management of Diabetes in the Primary Care Practice
By referring a patient to The Diabetes Center you can rest assured we will:
- Communicate treatment plans and outcomes to you
- Make your patient easier for you to take care of by helping them understand their role in diabetes management
- Alert your patient to diabetes complications and measures to prevent and diagnose complications
- Provide 24 hour access for patient's questions about their diabetes relieving the PCP of frequent calls
- Provide your patients with the support (emotional and physical) to successfully manage their diabetes
Referring Physicians should advise patients coming to the Diabetes Center to be sure to bring:

| Blood glucose meter and log |
| All medications in original bottles or a list of medications |
| List of questions and concerns |
