What should I bring to my appointment?
You should bring your medical records, including x-rays, lab results, and any notes from your referring physician as well as your insurance information and referral forms, if applicable. Please bring a list of all medications you are taking (names, dosages, frequency) and cash to utilize parking services. After scheduling an appointment, you will receive a medical questionnaire in the mail or through email. Please complete this questionnaire before your appointment and bring it with you to our office.
