When should I arrive for my appointment?
For an office consultation, you should arrive 15 minutes before your scheduled appointment. For a procedure, you should arrive 60 minutes before your scheduled procedure.
What should I bring to my appointment?
You should bring your medical records from your referring physician and, if applicable, your referral form. Please bring your insurance card, a list of all medications you are taking (names, dosages, frequency), and cash to utilize parking services. After scheduling an appointment, you will receive a new patient packet in the mail. Please complete this packet before your appointment and bring with you to our office.
