When should I arrive for my appointment?
For an office consultation, you should arrive 15 minutes before your scheduled appointment. For surgery, you should arrive 60 minutes before your scheduled surgery.

What should I bring to my initial consultation?
You should bring your medical records from your referring physician and, if applicable, your referral form. Please bring your insurance card, any relevant X-ray, MRI, myelogram, or CT films, and a list of all medications you are taking (names, dosages, frequency). You also should bring payment for any co-pays or co-insurances and cash to utilize parking services.
