What should I bring to my appointment?
Please bring recent laboratory values, blood glucose meter and log, and all medications you are taking in the original bottles (or a list with names, dosages, frequency). Also, bring a list of questions or concerns, cash for parking, and if applicable, insurance referral form.
Can my family members come to my appointment with me?
Yes, family members are encouraged to accompany you to your appointment.
How do I get to The Center for Endocrinology?
See Driving Directions to The Weinberg Garage at Saratoga. The Center for Endocrinology is located on the Lower Level accessed from the North elevators in The Weinberg Garage.
Where should I park?
You may park in The Weinberg Garage at Saratoga or valet your car in front of The Weinberg Center. See Parking Information for more details.
Does The Center for Endocrinology accept my insurance?
Please call your provider to confirm that The Center for Endocrinology participates with your plan.
Do I need a referral for my appointment?
This depends on your insurance coverage. Please check with your insurance company. If in doubt, contact The Center for Endocrinology at Mercy and we will assist you.

It is important for the physician to be aware of any changes and additions to your health history since your last appointment. These changes can sometimes affect your diagnosis and treatment plan.
